REPORTING A TABLET FAILURE


If you have a failed tablet and need to replace it or have it repaired:


Go to the IT Service Portal:

1. Access the Service Portal via the Hub, look for Quick Links, then select IT Help Desk

2. At the Service Portal, click on Report Incident

3. On the Submit a ticket form, fill in all the fields with a red *

4. In the Description field put the asset number of the tablet along with any other details about the problem. If you are replacing the tablet with a spare, also put the asset number of the replacement tablet.

5. Once you have completed the form, click the Submit button at the bottom of the form.

6. Your ticket will be submitted to the help desk.

7. Send the tablet to the IT Service Desk at the Modesto Annex.
